New Patient Registration
You can now register with our surgery online instead of popping in to pick up a pack of forms!

To be eligible to register at the practice, you must live within the practice catchment area.
Other ways to register
You can attend either of our surgeries to collect a New Patient Pack which you will need to take away to complete. Please read the paperwork carefully before starting to fill in. Your current practice will be able to tell you your NHS number if you do not know this.
You do not need proof of address or immigration status, ID or an NHS number.
Please return your completed forms to our Seaforth surgery. At that time you will be given an appointment for a New Patient Check.
If you are on repeat medication please ensure you have plenty of medication from your current practice before registering with us, as it will take some time for us to get your medical records.
